To whom it may concern
While visiting l'Hôpital de Gatineau for colonoscopy in 2021, I was shocked to see "65 DE LUCERNE" on my patient identification bracelet.
I used to live on "rue de Lucerne" in Gatineau, QC, from 1987 to 1988, and I visited Hôpital de Gatineau once or twice during that time. The street name changed to "rue de Toulouse" in municipal amalgamation in 2002.
Now that there is decent Québec Health Record (QHR) which is contains physical address, please get the correct address by cross-referencing the numbers instead of using a 33-year-old address.
