To whom it may concern

My dentist had determined that I had Periodontitis, and required deep cleaning of teeth under the gumline. The dental clinic checked with Sun Life on-line, as it usually does, but I had to pay the full amount.

Here is what CDCP/RCSD website says about Periodontal services.

cleaning under the gumline
treating abscesses
bonding for mobile teeth (requires preauthorization)
post-surgical evaluations (requires preauthorization)

Sun Life is administering the Canadian Dental Care Plan (CDCP) on behalf of the Government of Canada. Please explain why I had to pay the full amount with exactly 0 % from CDCP. Is it possible that wrong family income was applied in calculating my CDCP coverage? Here is my Member ID: 333333-XXXXXXXXXXX
